Who we are

It does not cover your dental and medical records, which are protected health information handled under HIPAA and our Notice of Privacy Practices, available at the front desk.
What we collect
Information you give us
- Appointment and contact forms: your name, phone number, email address, the treatment you are interested in, preferred times and days, how you heard about us, and anything you write in the message box.
- Calls and texts: when you call the office, your number and what you tell us so we can book you.
- Reviews and messages you send us through Google or social media.
Information collected automatically
- Analytics: we use Google Analytics 4 through Google Tag Manager to see which pages are visited, roughly where visitors come from, and whether buttons such as “call” or “book” are used. Google sets cookies to do this. IP addresses are not stored by us.
- Advertising: Google Ads conversion tracking tells us when a visit that started from one of our ads led to a call or a form submission. Google may use cookies to show you our ads on other sites (remarketing). You can opt out at adssettings.google.com.
- Server logs: our hosting provider records standard technical data such as browser type, pages requested and the time of the request, kept for security and troubleshooting.
How we use it
- To answer your request and book, confirm, change or remind you of an appointment.
- To call you about that request, if you agreed to be contacted by text when you submitted a form. Message and data rates may apply; reply STOP to any text to opt out.
- To understand which pages help patients, and to measure our advertising.
- To meet legal, insurance and record-keeping obligations.
We do not sell your personal information, and we do not use website form data for unrelated marketing without asking you first.
Who else sees it
- Google (Analytics, Ads, Tag Manager, the embedded map and any YouTube video you choose to play). Each is governed by Google’s own privacy policy.
- Trustindex, which displays our Google reviews on this site and loads content from its own servers when the reviews section comes into view.
- Our website host and email provider, which process form submissions and email on our behalf.
- Payment and financing partners such as CareCredit and Cherry, only if you apply for financing, and only what you give them directly.
- Authorities, if a valid legal request requires it.
Cookies
This site sets analytics and advertising cookies from Google as described above, and functional cookies needed for the site to work. You can block or delete cookies in your browser settings; the site still works without them, but we cannot then measure ads or analytics.
How long we keep it
Form and email enquiries are kept as long as needed to handle your request and for our records, then deleted. If you become a patient, the information becomes part of your dental record and is kept for the period California law requires. Analytics data is retained by Google under the retention setting of our Google Analytics account.
Your choices
- Ask us what personal information we hold from this website, correct it, or delete it, by calling (530) 406-1730 or emailing sldfrontdesk@gmail.com. We will confirm your identity first.
- Opt out of texts at any time by replying STOP.
- California residents have rights under the California Consumer Privacy Act to know, delete and correct personal information, and not to be discriminated against for exercising them. We do not sell or share personal information for cross-context behavioral advertising.
- Requests about your dental records follow HIPAA and our Notice of Privacy Practices, not this policy.
Children
This website is not directed at children under 13, and we do not knowingly collect their information online. Parents and guardians book on behalf of children.
Security
The site is served over HTTPS, form submissions are sent to the office by email, and access to those messages is limited to staff who need them. No method of transmission over the internet is completely secure, so please do not include detailed medical information in a website form; tell us by phone or at your visit instead.
Changes
We update this policy when the way the site works changes. The effective date at the top shows the current version.
